From July 15 till August 5, daily from 09.00 to 17.00 (except Sunday; on Saturday 09.00-15.00), the Working Group on \"Transfer and Restoration of Students\" will begin its activities in the \"Test Center\" of Tashkent Pediatric Medical Institute. Applications will be considered by the Working Group on August 27, 2019 at 09.00.

 Documents for submission to the Working Group on \"Transfer and Recovery of Students\"

 1. Application addressed to the rector (the application must contain the address of the current place of residence, the names of the parents, their phone numbers, their own phone number);

2. A copy of the passport, indicating the place of permanent residence (registration);

3. Rating book (the original must be filled out completely);

4. Extract from the order on academic leave, dismissal from the student’s number (a separate statement is issued for each order);

5. A copy of each page of the completed rating book;

6. Help from the dean of the relevant faculty (you must specify the difference in subjects and hours of study);

7. Certificate of activity from the moment of dismissal to the present day (from work, from the place of residence, from the family clinic, etc.);

8. Envelope (attached to the inside of the folder for the rating book);

9. folder;

10. Title page (name of the institute, faculty and course for restoration, information about education on a grant or paid contract basis, year of restoration).
