Brief information on the scientific project ADSS - 30.4 "Development of clinical and immunological basis of the forecast development , course and outcome of allergies in children , new approaches to treatment ."

Applied project aims at addressing the problem of early diagnosis of allergies in children , and improved methods of treatment. Accordingly, the purpose of this research is the development of clinical and immunological basis of the forecast development , course and outcome of allergies in children , new approaches to treatment . To achieve the goal to solve the following tasks:
• Examine the history and identify risk factors for the development of allergic diseases in the patients examined .
• Conduct analysis of genealogical pedigrees of children with allergic diseases .
• Identify features of clinical and functional manifestations of allergy examined patients .
• Set the prognostic value of the cytokine profile in determining the risk of development, course and outcome of allergy examined patients .
• To test and determine the efficacy of a new domestic selective oral leukotriene receptor antagonist " Monteksa " in the treatment of bronchial asthma in children.
• Develop risk of prognostic criteria , course and outcome of allergic diseases in children .
Benefits of development are predictive approach to identifying allergic diseases in children . Unlike the existing works, this paper aims to identify prenosological allergy symptoms in children. This will provide an opportunity to establish clinical and immunological criteria for prediction of allergic diseases in children that has not been developed to date. Timely diagnosis will allow for corrective therapy and prevent an adverse outcome . Complex therapy of allergies in children new domestic selective oral leukotriene receptor antagonist " Monteksa " will lengthen the remission of the disease , avoid p chronization process , disability patients and improve their quality of life .
Over the period 2012-2013 . project done : We included 121 patients with allergic diseases in age from 6 months to 15 years . Studied the chronology of the development of allergies, genealogical analysis of pedigrees , a retrospective analysis of health and physical assessment of the present status, the results of laboratory and instrumental methods ( cytokine profile , allergotesty , spirometry, PEFR ) . In patients with bronchial asthma tested and studied the effectiveness of domestic drug - leukotriene receptor antagonist " Monteksa " by 4 -point system . Based on sequential analysis of prognostic factors Wald designed to determine the likelihood of development, course and outcome of allergies in children . During the reporting period 5 published journal articles , 14 abstracts , one of them foreign. A lecture at the seminar «Pediatric Pulmonology» Salzburg April 27 - May 4, 2013. on «Quality of life of children with bronchial asthma».

The 2015-2016 school year the Department has the following staff: associate Professor Karimova М.Н. as acting head of the Department, associate Professor Zhuraeva З.Е.,candidate of medical Sciences assistant Koshchanova GA, Tuxtaeva O.T., assistants Babanazarov B.N., Ubaydullayeva О.Х., Aripova D.R., Artikova M.A. The main Department is stationed on the basis of city the children's cardiological center Shoyhontour district, there is an extra base in the clinical children №1. The head of cabinet to January 2016 was appointed Yakubova D.M, Master's degree in the direction of Pediatrics in 2015.

Project title ADSS 30.4 .: "«To work out clinic-immunological bases of prognosis, cause and outcome of allergic diseases in children and approach in new way of treatment»."

Data on the research project.

1.1 Development of clinical and immunological basis of the forecast of development, course and outcome of allergies in children, new approaches to treatment.

1.2 Applied research

1.3 PPI - 11 Protection of public health through the development of new technologies and methods of diagnosis, treatment and prevention of diseases.

1.4 Medicine and Pediatrics.

1.5.Gone XX century has been rightly called the "century of allergies." The frequency of allergic diseases is increasing rapidly - every 10 years, increasing by 10%, and in the world today, according to some estimates, up to half of the population suffers from various allergic diseases. According to the World Health Organization, more than 300 million people worldwide suffer from bronchial asthma (BA), and their number is growing, both among adults and children alike. Each year, the disease claims the lives of 100 thousand people. The average mortality data obtained from 48 countries, corresponding to 7.9 per 100,000 population. In recent years there have been numerous references to the increase in the frequency, severity, and "rejuvenation" of allergic reactions. Considering the cause of the "rejuvenation" of allergic diseases, the researchers attach greater importance to the impact of adverse factors during fetal development and early postnatal ontogenesis. Another important aspect of the problem of allergic diseases - a transfer of their child inherited. If you have allergies in both parents the risk of developing allergies in the unborn child is 60-80%, one of the parents - 50%. The risk of development of allergic disease in children of healthy parents may reach 10-20%. Data from numerous studies suggest that the presence of allergic disease in the mother is more likely than his father, it is an important risk factor for asthma in children. The special role of maternal effects on inheritance of atopy was called "Carter effect": the presence of atopy only 48% of maternal siblings had evidence of atopy, while 38% was observed BA; In the presence of atopy only father atopic symptoms were detected in 33% of children, and asthma - 25%.

In recent years, it was obtained surprising evidence that the immune system of the fetus as it is set up, prepared to develop allergies. Half (on his father's) foreign to the mother's body to the fetus in order not to be detached, stimulates the development of some of their lymphocytes - key cells of the immune system - in a direction favorable for allergies. This shift is particularly pronounced in children of parents with allergies. However, the alarming fact that often the diagnosis of allergic diseases, including and asthma is significantly delayed, which significantly worsens the course and prognosis of the disease. This dictates the need for improvement and the search for new treatments for this disease. Recently there have been publications on the appropriateness and effectiveness of the use of in the treatment of allergic diseases in children selective leukotriene receptor antagonists.

In connection with the above defined goal: to develop the clinical and immunological basis of the forecast of development, course and outcome of allergies in children, new approaches to treatment.

To achieve the goal to solve the following problems:

• Study the history and establish the chronology of "allergic march" have examined patients.

• Identify the features of the clinical and laboratory manifestations of allergic diseases

the surveyed children.

• Determine the diagnostic and prognostic value of the key indicators of cytokines in the patients examined.

• Develop risk prognostic criteria, course and outcome of disease.

• test and establish the efficacy of the selective oral leukotriene receptor antagonist "Monteksa" in the treatment of allergies in children

The main stages of implementation:

January - December 2012 - a survey of 40 children with allergies (food allergy and atopic dermatitis) in treatment allergy department at Children's Hospital №1 of Tashkent.

January - December 2013 - a survey of 40 children suffering from allergic diseases (allergic rhinitis, bronchial obstruction).

January - December 2014 - a survey of 40 patients with bronchial asthma, the analysis of the effectiveness of the therapy, statistical processing of the material.
